May 2, 2007, 10:37 p.m.The study adds yet another frustrating twist to the back-and-forth findings about whether hormone-replacement therapy protects against diseases of aging. Though the accepted answer has been "no" in recent years, the latest evidence suggests that timing of treatments may be key, at least for heart attacks and now for dementia.

May 3, 2007, 12:36 p.m.We now know the risks associated with HRT. We know that not every woman needs it, and the ones that do should use the minimal dose necessary to relieve symptoms.
As for dementia, not everyone will have significant pathological neuronal changes as they age. The key will be determining who is at risk, determining effective dosing, and monitoring closely.
Hopefully there will be genetic markers that will help us determine who is at risk, and improved assessment tools to diagnosis earlier onset of dementia. As usual, the earlier the appropriate intervention is achieved the better the outcome.
