"...the leading cause of liver failure in the U.S. and sends 56,000 people to the emergency room annually. About 200 die each year."

Ahhh, the irony.
The toxicity of Tylenol and the like has long been known by medical professionals. As a matter of fact, many hospital ER doctors won't even prescribe it because they don't want to introduce it into their patients' medication "repertoire" because the potential for abuse is too high.
That said, all drugs are toxic at some level and one must take the time to read and follow ALL directions on the package and NEVER take anything with alcohol. The potential damage to one's liver with the combination of acetaminophen and alcohol is all but a sure thing...if not the first time, then the second , third, fourth, etc...whenever it occurs, it won't be worth the risk.
While the FDA has taken entirely too long to address this issue and health providers, for some reason, haven't made a point to inform their patients, the days of completely trusting one's own health with others are long gone...if they really even existed in the first place.
You have to be your own advocate...which means becoming as familiar as you can with both your condition and treatment - one doesn't need to be a genius to understand medicine.
That said, I understand Zgirl's concern (below) and I too wonder if it's really necessary to make it prescription-only. It's all about following directions and unfortunately, too many people choose to gamble with their own health by self-prescribing ..."if a larger dose didn't harm me the first time, it must be safe for me.." However, many drugs build up over time in the body, so for those who have this attitude toward medication, it's just a matter of time for many.
Sorry - sore subject. End of rant ;)-

another thing, quicksilver, - i thought my daughter had an allergic reaction when I gave her tylenol for her fever, and the doctor said it was almost IMPOSSIBLE to be allergic to it, that is why is so widely recommended. He said in his 38 years of practice, he never once saw someone have an allergic reaction to Tylenol.
